*If you are applying for a seat that is Infant to PK-3, some funding sources require birth certificates from all children in the household. Please include those on the application to ensure you have every possible opportunity to receive a free seat.

Parent(s) not listed on proof of age document must provide proof of custody

• Custody Judgement(s) Or
• Child Placement Agreement from DCFS Or
• Provisional Custody by Mandate Or
• Military Power of Attorney Or
• Non-legal Custodian Affidavit

To verify household size, parents must provide ONE of the following for each child under the age of 18 living in the same household as the applying child:

• Birth certificate
• Current passport
• Visa.

If the parent/guardian’s name is not listed on the birth certificate, additional documentation proving legal guardianship is required.

• Name on identification submitted must match name of person submitting NCAP

• Louisiana Driver’s License OR State-issued ID Card
  • If you used your Louisiana Driver’s License OR State-issued ID Card in to prove guardianship, it cannot be used to prove residency.
• Current Lease OR Mortgage Statement
• Water Bill
• Entergy Bill
• Cable Bill
• Either the parent/guardian name must be on the residency documents or if the parent/guardian lives with another adult who is named on the residency documents, the parent/guardian must upload one proof of residency in the resident's name, a photo ID from the person named on the documents, and a letter of living arrangements from the named person.

Used to Verify

• Adult name on NCAP matches proof of address documents (if not submitting letter of living arrangements)
• Family lives in New Orleans or Louisiana

Children with disabilities and children whose home language is not English may be eligible to receive services but must still qualify to participate in publicly funded programs. These children may be able to be served through other funding sources such as Title I, 8(g), and IDEA. 

Not categorically eligible, submit the following:

• Birth to 3 Years 
  • 4 consecutive pay stubs from the last 45 days for ALL parents and other adults in the home including Social Security Income benefits, if applicable, accompanied by all other applicable income documentation Or
• 4 Years 
  • 2 consecutive pay stubs from the last 45 days for ALL parents and other adults in the homeincluding Social Security Income benefits, if applicable, accompanied by all other applicable income documentation Or
• A statement from the Social Security Administration verifying that the child listed on the application is a recipient of SSI benefits, which must be accompanied by two current pay statements Or
• Official employer letter stating place of employment, hourly rate of pay & average number of hours worked per week Or
• Unemployment Benefits – must submit a Monetary Determination letter from the Workforce Commission Or
• Statements of No Income form, if claiming zero income Or
• Declaration of irregular Employment, if intermittently or self-employed with no other proof of income to provide
• If applicable, please submit a SNAP/Food Stamps benefits statement letter. This letter must include the parent/guardian’s name, the child's name, and valid effective dates. Additionally, if you have a job and receive SNAP benefits, please provide proof of both.

*Statement of No Income and Declaration of Irregular Income information is now available in the application in multiple languages

• Total household gross income based on hourly rate of pay

*Unemployed families may qualify for multiple funding sources and are required to submit proof  of seeking work/job training via the Louisiana Workforce Commission’s HiRE System.
